The Ministry of Community and Social Services provides additional family law forms that are required in certain circumstances in accordance with government policies and legislation. Supreme Court rules and forms govern Supreme Court proceedings throughout Ontario. Each of the following areas of the Court has its own specific rules and forms. This form is required in Section F of Part V of the Consolidated Provincial Practice Branch of the Court. The Code of Civil Procedure requires the use of a number of mandatory forms for civil proceedings. This form can be used as a confirmation project to provide guidance on the procedure to be followed in these cases. The new Process Planning Confirmation Form is required before a negotiation date is specified in a family case. Litigants are asked to complete Parts 1 and 2 of the form for or immediately following the settlement conference. Part 3 of the form is completed by the judge after the issues have been discussed with the parties. .
